We’re excited to provide a week of fun, creativity, and movement through activities such as art, dance, games, and outdoor winter play. Before your child participates, please complete this waiver and permission form. This helps us gather important health and emergency information, obtain consent for supervised activities, and outline our camp policies. Activities may include indoor programming as well as outdoor play in winter conditions (weather permitting). Your submission confirms that you understand and agree to the terms below.
